Enter the FCC Registration Number (FRN) of the Assignee/Transferee. The FRN is a 10-digit number that is assigned to all businesses and individuals who do business with the FCC. You can access the Commission Registration System (CORES) to search for the FRN by selecting the CORES link. For more information on FRN, access the Help topic Login/FRN in the Help navigation menu.
Select the radio button corresponding to either "Assignee/Transferee name (if entity)" or "Assignee/Transferee name (if individual)." If you selected to enter an entity's name, enter the name in the space provided. If you selected to enter an individual's name, enter the First Name, Middle Initial, Last Name, and Suffix in the appropriate boxes.
Note: Do not use punctuation marks in any of the boxes provided. For example, "John Q. Smith, Sr." should be entered as "John Q Smith Sr" on this page.
If the Assignee/Transferee is an individual, first and last name are required.
If the Assignee/Transferee is an entity, the Assignee/Transferee Name field is required.
The Attention To field is required.
Enter the name and FRN of the Real Party in Interest. Click the Pre-Fill button at the beginning of this section to pre-fill the Name and FRN of the Real Party in Interest with the Assignee/Transferee Name and FRN that is entered in the sections above this one. You can access the Commission Registration System (CORES) to search for the FRN of the Real Party in Interest by selecting the CORES link. For more information on FRN, access the Help topic Login/FRN in the Help navigation menu.
Complete all fields in this section which apply. You must enter either a P.O. Box or a Street Address. You must enter the City. You must select a US state or territory from the pull-down menu and then enter a valid zip code.
The Phone Number is required and must be 10 digits.
The demographics questions are optional. Select the check-box next to each race description that applies. Choose from the following race descriptions:
For ethnicity, select from the pull-down menu either "Hispanic or Latino" or "Not Hispanic or Latino." For gender, select "Male" or "Female" from the pull-down menu.
To clear all information from the page and begin again, click Clear at the bottom of the page.
